Student Handbook
The Holy Name of Jesus handbook includes both school and archdiocesan policies. Every school family is expected to read through the handbook and return an acknowledgment form to school. If you did not receive a Handbook or would like to review the Handbook, please stop by or contact the school office.
School Forms
Please click on a link below to view/print a PDF of the form. For your convenience, several of the forms are “fillable” PDFs, which means you can open the PDF, type information onto the form and save the PDF. Once the PDF has been completed, there are several ways you can send it to the school: (1) email the PDF to the school as an attachment to admin@hnojla.org, (2) print the PDF and mail it to the school office, or (3) print the PDF and drop it off at the school office.
